Cymbacephalus nematophthalmus inhabits sandy areas near rocks, weeds and mangroves usually shallower than 10 meters before.
It is a mottled grayish brown flathead with 7 - 8 dark bands across the back and sides, its fins are spotted with light and dark spots, the dorsal and caudal fins with short dark stripes or diagonal lines.
There are 6 - 9 skin lobes above each eye, a broad dark band below the eye, and large branched iris lobes distinguish the head area.
The upper side of the crocodile fish is gray-green to brownish or blackish, and its underside is whitish,
Cymbacephalus nematophthalmus differs from the similar Cymbacephalus staigeri (Castelnau, 1875) in having distinct eye patches and one or two suborbital spines (as opposed to lacking eye patches or suborbital spines in Cymbacephalus staigeri) and a shorter snout in Cymbacephalus nematophthalmus).
The two species also differ in coloration.
The specific species name is composed of the Greek "nemato"- (= thread) and "ophthalmos" (= eye), probably in reference to the thread-like eye patches of this species.
